I hereby authorise My Gym to charge my credit/debit card every two (2) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.
All members will have a `Home Class` for their child recognised in the system as their usual attendance day. For members on the `once a week` membership, if a child is absent, makeup classes can be arranged through correspondence with staff via call, text or email. These makeup classes will be held on the account for a maximum of 4 weeks until arranged, otherwise they will expire. For unlimited members, there are no make-up classes kept on the account for missed classes since the arrangement permits attendance on any day of the week to offer flexibility for however many classes they`re able to attend (limited to 1 class a day). There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrolment and receive an email confirmation from the gym stating your cancellation request has been processed. The cancellation must be done in Writing by either filling out a Cancellation Form or via Email, and we request at least 2 weeks notice, (or a final payment notice). It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
Membership and tuition are nonrefundable and nontransferable.
