After my initial trial period, I hereby authorize My Gym to charge my credit/debit card every four (4) weeks up until a cancelation is filled out through my online account. I also authorize a 1-time-life-time membership fee of $75 unless I am already a My Gym Member. The current tuition is stated on this form and is subject to change with notification.
If I do not wish to continue after my 1-week unlimited guest experience period it is my responsibility to cancel the auto-enrollment via online system. Speaking to a teacher about cancelling my 1-week guest experience during classes does not constitute an auto-enrollment cancellation notice.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment online and receive an email confirmation from the gym stating your cancellation request has been processed. All cancellation requests must be completed online via your online account. It is agreed that, upon cancellation, you will continue to attend the gym until your final day of membership (My Gym requires a 30 days minimum notice and are only accepted via online account). It is my responsibility to attend the remainder of my classes in my contract. My Gym does not offer any refunds for missed classes. If there is a tuition payment within your final 30 days of class that tuition payment will be prorated for the number of weeks left in your contract. Any makeup credits in your account should be completed before your final class. These makeup credits will expire on the date of your last scheduled class as per your contract.
Please note: speaking to a staff member, texting, or emailing does not constitute a cancellation. All cancellations must be submitted online by an authorized account member and must include an e-signature from the authorized member on file.
Members agree to remain continuously enrolled until terminated by the member via online account. A Fee of $30 will be charged for all returned payments. A $30 fee will be charged for late payments by check or cash, or due to denied card authorizations. Denied cardholders have 3 business days to provide valid card information. Any and all payments to My Gym and memberships are non-refundable and non-transferable under any circumstances.
I agree to pay the above amount to My Gym. I understand that all payments made to My Gym are nonrefundable under any and all circumstances. Store credit for classes missed due to severe illness or injury may be given at the sole discretion of MY GYM management.
I understand that all membership fees and deposits made to secure a Birthday Party date are non-refundable and non-transferable in all circumstances. A Birthday Party can be rescheduled without the loss of deposit. Rescheduling requires an additional fee and is contingent upon calendar, staff availability and My Gym Management discretion.
Should I dispute a charge through my financial institution will constitute a breach of contract which may result in, but is not limited to, penalties, additional fees, collections, legal action, and/or termination of any and/or all current and future services.
