I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend a make-up class before or after the closure. All classes must be scheduled in advance online, space permitting. Make-ups must be completed while member is currently enrolled at My Gym Kent and expire once an online cancellation has been submitted.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment and receive an email confirmation from the gym stating your cancellation request has been processed. All cancellation requests must be completed online via your online account. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership. Please note: speaking to a staff member, texting, or emailing does not constitute a cancellation. All cancellations must be submitted online with an e-signature from the member on file.
A Fee of $15 may be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
