I hereby authorize My Gym to charge my credit/debit card on the 2nd of each month in the tuition amount stated above.
Membership includes 1 structured class per week and unlimited access to practice & plays. Member must notify MY GYM prior to missing a class. Make ups must be done within 2 weeks before/after a missed class. Please, one makeup per child per month. Tuition is based on a 4-week program. The months that offer a 5-week program compensate for the holidays and camps when the gym is closed. There are no refunds for unused classes or practice & plays.
Your child automatically remains on the class roster until you notify MY GYM in writing that you are withdrawing your child. This agreement is a continuous payment plan, and will continue until buyer terminates agreement. You the buyer may cancel this agreement by sending a written cancellation notice to the club, thirty (30) days prior to the payment due date on the agreement. This notice must be sent via certified mail or by coming into the club and filling out a cancellation form. All membership dues payments must be brought current at the time of cancellation.
Members agree to remain enrolled for a minimum of two (2) consecutive, recurring payments. A Fee of $10 will be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
Multiple Class Discounts - 0% off additional classes for the same child
This account already has children enrolled in a class or trial. Proceed only if you'd like to enroll and pay for an additional program. If you want to schedule an Unlimited Class, Makeup Class, Drop-In or a Class Card please visit the Schedule Classes tab in your account.
