TERM:
I hereby authorize My Gym Ottawa to charge my credit/debit card every four (4) weeks.
The current tuition is stated on this form and is subject to change with notification.
Tuition is based on four (4) weeks of class programming. Members must schedule their unlimited classes via their online my gym account.
There may be short periods when My Gym is closed or classes are not running. For those days an alternate makeup online class time will be scheduled.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and remain in effect until terminated by the member. As per my Membership Agreement, I understand I am responsible for any past due amounts. This agreement will terminate on the final day of my membership.
I understand that I may attend the gym until the final day of membership. No refunds or credits will apply for any missed classes.
Membership and Tuition are nonrefundable and nontransferable
