I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absence a minimum of 2 hours in advance to be eligible for a make-up. Each make-up must be completed within four (4) weeks of the missed class. There may be times when desired classes are full. We guarantee you a space in the class(es) for which you are registered only. There may be short periods when My Gym is closed or classes are not running. If this occurs and a class is missed, the member is entitled to a make-up. All make-ups must be completed by your final class date.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment and receive an email confirmation from the gym stating your cancellation request has been processed. The preferred method to request a cancellation is to do so from your My Gym account, but we will accept email as well. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
Membership and tuition are nonrefundable and nontransferable.
