I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with notification.

I hereby understand that My Gym holds the rights to apply service charges to any refunds if applicable. I am aware that currently there is a $5 charge for tuition refunds on monthly payments and $20 charge on packages, which can be subject to change at any given time. I recognize that My Gym classes are non-refundable after attending the first class for all monthly classes, packages and class cards. Refunds may only be applicable in extreme cases such as severe sickness, personal loss, or major relocations accompanied by written documentation.

Tuition is based on four (4) weeks of active enrollment that at least attend 1 class or more than 1 class based on availability for unlimited core gym classes, this include Tiny Tykes, Waddlers, Gymsters, Terrific Tots and Mighty Mites. There is no compensation or make up class available for unlimited enrollment. If you miss a booked class, you can reschedule that class on other days or other weeks based on class availability.

For programs such as Ninja Training, Preschool Prep and Gym& STEAM, tuition is for every 4 classes. Families must inform the gym of an absence a minimum of 2 hours in advance to be eligible for a make-up. Each make-up must be completed within four (4) weeks of the absence. Policies differ for Learn, Create & Explore and other ancillary programs. Please ask a My Gym representative for details.

This agreement constitutes a continuous payment plan on a 4-week ongoing basis and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until an official Cancellation Request Form (CRF) has been completed and submitted online or in person. A 2-week cancellation notice is required. You are able to select a last day for your classes online at any given time. Class payments and packages are non-refundable after the first class of the payment period. This agreement will terminate after the last scheduled class. Children will continue to attend classes and Practice and Plays as well as complete any remaining booked classes through that date.

Sibling Discount - 10% off additional children enrolled
Twin Discount - 20% off the second twin
Triplet Discount - 20% off the second and third triplet

Multiple Class Discounts - 10% off additional classes for the same child
