I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with notification. The New Member Discount is a promotion that is applicable to your first 3 months of membership. Beginning the 4th month, tuition will increase to the regular gym tuition rate

Tuition is based on four (4) weeks of class programming. This includes unlimited age-appropriate classes, and practice and play (open gym) times throughout the week. These classes must be scheduled in advance as `unlimited` through your MY Gym account. The class you have registered for is your home class and will be the main attendance you are on weekly, no need to `schedule unlimited` every week for that class. Please inform the gym of an absence a minimum of 2 hours in advance by `marking absent` on your My Gym account. Because we offer unlimited classes, `make ups` are not part of our program. We do allow members to freeze their membership in the case they have an upcoming vacation or event. Freezes can be placed for a minimum of 1 week, and a maximum of 8 weeks. There may be short periods when My Gym is closed or classes are not running. If this occurs and a class is missed, your payments will pushed to the following week.

This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until we have received your cancellation request via your My Gym account.

To cancel your membership:

Log into your account at www.mygym.com/riverforest/account
Click `Schedule Classes` then your child`s name
Click `Cancel` or `Opt Out`
Select the reason
Sign & click Submit

You’ll receive a cancellation email. Let me know if you have any other questions!It is agreed that, when a cancellation request has been received, the member will be remain in class until 28 days after date of cancellation. Any payments may be prorated to reflect this if necessary.

Members agree to remain enrolled for a minimum of two (2) consecutive, recurring payments. A Fee of $15 will be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.

Sibling Discount - 30% off additional children enrolled
Twin Discount - 50% off the second twin
Triplet Discount - 50% off the second and third triplet

Multiple Class Discounts - 30% off additional classes for the same child

This account already has children enrolled in a class or trial. Proceed only if you'd like to enroll and pay for an additional program. If you want to schedule an Unlimited Class, Makeup Class, Practice & Play or a Class Card please visit the Schedule Classes tab in your account.
