I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification. The New Member Discount is a promotion that is applicable to your first 3 months of membership. Beginning your 4th month, tuition will increase to our regular gym tuition rate.
Members must inform the gym of an absence in advance. There may be a short period when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment. All cancellations must be submitted on your My Gym online account. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
Membership and tuition are non-refundable and non-transferable.
