I hereby authorize My Gym to charge my credit/debit card every 4 weeks (every 28 days) on a recuring basis. The current tuition is stated on this form and is subject to change with notification.
If enrolling in a trial I acknowledge and authorize My Gym to charge my card the current tuition cost on the day my trial is scheduled to convert to enrollment.
I acknowledge that cancellations require 30 days notice and must be initiated and signed using my online account.
Tuition is based on four (4) consecutive weeks of enrollment. Members must mark themselves absent in their online account to be eligible for a make-up. Each make-up must be used in a class other than your enrolled class. They have no monetary value.
There may be short periods when My Gym is closed, or classes are not running due to inclement weather or holidays. If this occurs and a class is missed, the member is entitled to a make-up, and payments are still be run on the normal schedule.
Make-up classes are a benefit of enrollment, they do not extend enrollment. All make-up classes must be completed by the last day of enrollment if a cancellation has been initiated.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until an official Cancellation Request Form (CRF) has been completed.
4 weeks` notice is required to cancel membership. Use your online account to initiate and sign a cancellation. My Gym staff cannot cancel any memberships in person or over the phone. It is agreed that, when a CRF has been received, the member will be responsible for any past due amounts. This agreement will terminate after the last scheduled class. All make-up classes must be completed by the last scheduled class.
There are no pro-rations or refunds for cancelling your membership after a payment has been made.
A Fee of $15 will be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
