I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification. New membership promotional rate of $99.00 for the first four (4) weeks for an unlimited membership. Beginning the 2nd Month tuition will increase to standard $129 for 1 weekly class or $149 for unlimited weekly class tuition.
Members must inform the gym of an absence in advance. If you do not inform us of an absence there will be no makeup class given. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment with a 28 day notice 1 full billing cycle and receive an email confirmation from the gym stating your cancellation request has been processed. The preferred method to request a cancellation is to do so from your My Gym account. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of payment . A signature is required for our merchant auto billing cancellation verbal and text cancellation is not accepted.
A Fee of $15 will be charged for all returned payments. Any and all membership payments to My Gym are non-refundable and non-transferable under any circumstances.
