I hereby authorize My Gym to charge my credit/debit card every four (4) weeks (every 28 days). I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. I understand 4 weeks` notice is required for cancelations and all cancelations must be completed in my online account. Staff cannot process cancellations verbally or in person. The current tuition is stated on this form and is subject to change with advance notification.
Tuition is based on four (4) consecutive weeks of enrollment. Members must mark themselves absent in their online account to be eligible for a make-up. Each make-up must be used in a class other than your enrolled class. They have no monetary value. There may be short periods when My Gym is closed, or classes are not running due to inclement weather or holidays. If this occurs and a class is missed, the member is entitled to a make-up, and payments are still be run on the normal schedule. Make-up classes are a benefit of enrollment, they do not extend enrollment as they have no monetary value. All make-up classes must be completed by the last day of enrollment if a cancellation has been initiated.

You will automatically remain enrolled at My Gym until you cancel your enrollment using your My Gym account. You will receive an email confirmation from the gym stating your cancellation request has been processed when it is completed correctly. My Gym requires 4 weeks` notice to cancel membership. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership. ALL MAKEUP CLASSES EXPIRE AS OF LAST MEMBERSHIP DATE. Makeup classes are a benefit of being currently enrolled. Makeup classes have no monetary value and cannot extend enrollment in any way if a cancelation is on file.
Membership and tuition are nonrefundable and nontransferable.
