I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. (When enrolling in a Trial, this agreement can be terminated anytime during the Trial Period by clicking on your child’s name, then clicking on Cancel Trial when using your online account.) I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. If child`s class day is changed, this will also change the next bill date to coincide with the new class day. The current tuition is stated on this form and is subject to change with advance notification. A Fee of $15 will be charged for late payments.

This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment and receive an email confirmation from the gym stating your cancellation request has been processed. The preferred method to request a cancellation is to do so from your My Gym account, but we will accept email requests sent to: [email protected]. Cancellation requests must be received no less than ten (10) days prior to your next billing date and you must receive an email confirmation to avoid future billing. Cancellations are not taken verbally to ensure we both have a “paper trail.” It is agreed that upon cancellation, you may continue to attend the gym until your final day of membership which will be (6) days from your last scheduled Home Class day.

Membership and tuition are nonrefundable and nontransferable. Once a tuition payment is made, your child`s class spot is reserved and there is no refunds or credits for missed classes.
