Trial Period Billing Policy: We use a convenient auto-enroll feature to activate your membership after your unlimited trial period. The day after your trial ends, you`ll become an active member at the tuition rate of $136.00/month for ONE class and unlimited practice & plays.
I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absences and schedule unlimited/make-up classes in their online account in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures, in addition no refunds for missed classes due to sickness/vacation/etc... To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure. One class membership: make-ups will be given for missed classes and expire in 8 weeks from missed class or upon cancellation, if MU is scheduled and you do not attend, the MU credit is forfeited. There are no make-ups given for missed Practice & Plays. Unlimited membership: will not receive make-ups for missed classes/practice & plays - as you may attend every age-appropite offering the following week.

You will automatically remain enrolled at My Gym until you cancel your enrollment and receive a confirmation from the gym stating your cancellation request has been processed. The ONLY accepted method to request a cancellation is to do so from your My Gym account, NO VERBAL/EMAIL/TEXT/NOTE cancellations will be accepted. My Gym requires a 30-day notice to cancel. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership that was paid.
Membership and tuition are nonrefundable and nontransferable.
