A $75.00 lifetime membership fee may be due depending on current membership status.
I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with notification.
Tuition is based on four (4) weeks of class programming. Members must inform the gym of an absence a minimum of 2 hours in advance to be eligible for a make-up. Each make-up must be completed within four (4) weeks of the missed class. There may be short periods when My Gym is closed or classes are not running. If this occurs and a class is missed, the member is entitled to a make-up. Policies differ for Learn, Create & Explore and other ancillary programs.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until an email is sent to
[email protected] or a text sent to (407) 654-6970. You can also submit your cancellation through your online account. It is agreed that, when this cancellation notice has been received, the member will be responsible for payment of all remaining classes up to four weeks after the next scheduled class. This final payment may be prorated based on your next scheduled payment date. This agreement will terminate after the last scheduled class. All make-up classes must be completed by the last scheduled class.
Members agree to remain enrolled for a minimum of two (2) consecutive, recurring payments. A Fee of $15 will be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
